What steps do I need to take to replace the upper door ...

What steps do I need to take to replace the upper door hinge on a 1997 Dodge Dakota?    Do I need to remove door panels etc. or are the nuts on the bolts through the door all that has to be removed from the door? How hard is it to reach the hinge bolt under the dash to remove the hinge from the door frame?
