transmiision diagnosis

I have a 1996 chevy k1500 z71 with extended cab,tranny has been rebuilt two times in 3week due to frist build natural wear and tear,now my problem is it shift fine from 1st to second but will not shift from 2nd to 3rd unless i get rpms up to 3500 if rpms drop it will not catch gear unless rpms are back up,no manual 3rd gear
had transmission rebuilt it ran fine 4 about two days and started to act out the same symptoms keeps given tcc code no # though,please were do i look next can it be adjustments orr shift sol.
