ROUGH ENGINE

my 96 e 150 cargo van idles rough and seems to miss when pulling uphill and accellerating. looking through the prev. owners service records, it has along history of rough idleing-
the van has a 351/5.8 motor and is a conversion van - any known reasons behind this problem? thanks!
