2000 Ford Taurus cold start engine stumble ...

2000 Ford Taurus with vulcan (2 valve) engine.
On cold start it runs rough, start driving like this and it surges, hesitates, and almost dies when depressing accellerator. The exhaust is very smokey.
When the engine is warm, it is fine.
When this first started, it had to be less than 25F outside to happen and letting run for 2 minutes, turning off and restarting, it would be fine. It has slowly gotten worse, the temperature at which the cold start has the problem continues to rise, presently about 75F. Also, it now takes about 5 minutes warming up to be able to key off and restart without issue.
The engine light is not on, there are no DTC's present.
Any experience or thoughts on fixing would be appreciated.
