A few months ago the fan on my 2000 Plymouth

A few months ago the fan on my 2000 Plymouth Grand Voyager SE would not turn off. I removed the fuse to check it when I put it back it seemed to work fine. Now the fan is not turning on at all and it starts to overheat if I am stopped or going slow for short periods of time. Is it the fuse or the fan relay and if it is the relay where is it located and how do I replace it?
