The first two things I would do are check for vacuum leaks(hoses and intake manifold gaskets)and then perform a compression test and an idle vacuum test.On the idle vacuum test you will be looking to see if the vacuum gauge needle is bouncing rapidly or drops every so often a good bit.Compression should be compared across all cylinders if you do the test and post the numbers I can then form an opinion.
