98 Avalon Fuel System Problem

I have a 98 Avalon that begins to run very rough after running at higher speeds, 60-70 MPH, for more than 5 minutes. It seems like the engine can't get enough fuel and the car will jerk back and forth. The tac will also race when this occurs.   The only way to stop it is to put the car in neutral and put the accelarator to the floor for a few seconds.

This has been been going on for nearly a year. I've had it into the Toyota shop a couple of times and my normal mechanic and neither can find the problem.   Until recently, the problem occured only at higher speeds, now it is starting to run rough a lower speeds as well.

It seems to me that it is fuel system related. At the last trip to the Toyota shop they did a scan and the codes P0300, P301, P205, 306, seemed ot indicate bad gas according to them. But the fact that this has been happening for so long, bad gas can't be the problem. Plus, I always put 92/93 octane in the car and use a number of different gas stations.

Any ides?
