I have a 99 honda accord automatic .. when im driving

I have a 99 honda accord automatic .. when im driving in between switching gears the rpms jump high and causes the car to jump into gear what could this problem be ?

Optional Information:
1999 Honda accord 2.3 vtec

Already Tried:
changed axles , motor mount in front ,new tires ,changed tranny fluid
